Fumbling the Handoff in Transition Care

The 25th symposium was held on Wednesday, February 20, 2019, 12:00-2:00 pm at Emory University.  The Keynote Speaker was Mary D. Naylor, PhD, RN, Marian S. Ware Professor in Gerontology and Director of the NewCourtland Center for Transitions in Health at the University of Pennsylvania School of Nursing.
